Recommends appropriate pharmaceutical prescription based on patient parameters and interpretation of current literature, safety, efficacy and cost; 

Advises prescribers on patients’ medication therapy; 

Assists in the Pharmacy and Therapeutics Committee process for review and evaluation of treatment options; 

Maintains accurate medication histories and monitors effect and adverse reactions of all patients; 

Participates in multi-disciplinary team activities to provide patient-specific medication information and education when appropriate; 

Evaluates information from prescriber or interviews patients and provides education or assistance for the self-administration of take-home or discharge medications;  

Assists in problem resolution with the healthcare team; 

Directs the work of and instructs pharmacy students, residents, interns and/or pharmacy assistants; 

Assists in the development, implementation, and evaluation of clinical pharmacy services such as clinical pharmacy, specialty residencies, workshops, and continuing education programs in the clinical service; 

Participates in development, implementation and evaluation of departmental policies; 

Participates in the development and documentation of quality assurance indicators such as drug use evaluation; 

Develops and presents periodic in-services to healthcare providers; 

Participates in pharmacy and hospital committees; 

Participates in research studies in the clinical service; 

Performs the duties of a Pharmacist 2;
Performs related duties as required.

Knowledge of: pharmacology, pathophysiology, pharmacokinetics, current medical literature and techniques of critical evaluation, clinical drug usage, patient monitoring techniques, drug blood level analysis and interpretation.
